- Name and phone number of your insurance company (sometimes located on the back of your insurance card)
- Policy number (located on the front of the insurance card)
- Group number (located on the front of the insurance card)
- Social Security number of insured (only if policy number isn’t on the insurance card)
- Patients’ date of birth
- Name of Insured
- Date of birth of Insured
- Your name and relationship to the Insured
- Does my policy have chiropractic benefits?
- Are there any specific limitations for chiropractic such as maximum number of visits per year, dollar maximum per year, dollar maximum per visit?
- Is there a deductible for chiropractic benefits?
- What is the percentage of coverage of spinal manipulations and x-rays after the deductible?

Do I need a referral from my doctor?
Pinhook Chiropractic is a participating member of many insurance plans. If we are not listed in your provider book, do not worry as most insurance companies have “IN & OUT” of Network benefits for chiropractic care. In addition, we continually join many new plans as they become available in our area so we always recommend that you ask us if we are part of your plan.

CareCredit is a personal line of credit for healthcare treatments and procedures for your entire family. It works like a credit card but it has two advantages. It can only be used for healthcare services and you can get No Interest* financing every time you use it. Simply pay your minimum monthly payment and pay off the entire balance by the end of your promotional period and you pay No Interest. If you need more time to pay for your treatment, you can take advantage of extended payment plans with low, fixed interest rates.
Medicare:
We are providers of Medicare Part B. As a courtesy, once Medicare processes your claims, we will also file your claims to a supplemental policy if you have additional coverage whether it be only a supplement or a full Secondary policy.
No Insurance:
For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. If you have no insurance or do not have chiropractic benefits, there is still a way for you to receive the care you need. Many patients pay directly for care, as they discover chiropractic to be extremely cost-effective and affordable.
If you are injured on the job, your care should be paid for under your employer’s Worker’s Compensation insurance. You will need to inform your employer of the accident, file a claim, and obtain the name and address of the carrier of their insurance. This information will need to be provided to our insurance department at the time of the initial consultation.
Please notify your auto insurance carrier of your visit to our office immediately. We accept and file claims to “Med-Pay” policies, “Liability” policies, and Attorney cases. Notify our insurance department immediately if an attorney is representing you. We do accept assignment, and payment for services rendered is due immediately upon settlement of the claim.
